Ask for quick help: I don't understand US addresses :/

Hi, I'm from Chile. Some days ago, a fellow member of this board sugested me that I could get a "casillero" from my home post service in order to buy in the US, send the goods to the casillero and from then ship them to Chile.

Well, I did it. Correos de Chile offers the service and I opened my casillero in Miami. The info is this one:

But I have this confusion: Addresses in my country have actual words! and then some numbers (mine, for example, is Agua Santa 1400 and the apartment number, pretty simple), but this thing is basically a really long code of numbers.

I want to use this as my address in Ebay, but I'm not sure what part is the street address, what is the zip code, and I prefer to ask before making a mistake

I think the street address is: 9990 NW 14th Street Ste 110, probably the address 2 is the casillero "CCH126378", then the city is Doral (or is it Miami), and the zip code is (786) 845 8828. Or am I wrong?
